Spontaneous intraparenchymal hemorrhage is a devastating acute illness with a changing epidemiology (more young cases because of drug abuse). Attempts to limit haematoma expansion have not been beneficial in terms of long-term outcome. Surgical haematoma removal (STICH trials) has been questioned.
