Primary ileal tumors are a rare occurence, as well as metastatic involvement. As far as head and neck tumors are concerned, the most frequent metastases involve the lungs, bones and liver, being ileal involvement extremely rare. We report a case of a 77 year-old man with squamous cell carcinoma (spinalioma) of the floor of the mouth and of the right anterior tonsillar pillar which was resected in1997 with emimandibulectomy, emiglossectomy and radical modified neck dissection. This patient presented, 9 years later, abdominal pain and bleeding, due to ileal tumor which underwent surgical resection. Histological examination revealed a metastatic squamous cell carcinoma from primary oral tumor. To our knowledge there are very rare reports of primary tumors from oral cavity metastasizing to the small bowel and presenting ante mortem.
