Background. The multidisciplinary standardized interventions to educate and support patients with congenital heart disease (CHD) are described as “Transition Clinic” (TC). TC represents a key element to deliver care for patients during the transition from childhood to adulthood. So far, there is a lack of empirical evidence regarding the impact of TC models on the improvement of health perception in adolescent patients with CHD (CHD-specific TC model). For this reason, the aim of this study is assess the impact of the TC model on CHD adolescent patients’ health perception outcomes. Methods. This study has a quasi-experimental design. Quality of life, satisfaction and health perception were assessed in T0 and after 1 year from enrollment (T1). During the follow-up period, the patients enrolled (aged 14 to 21 years) were involved in the CHD-specific TC model. Results. The results are referred to the first 100 patients enrolled (mean age 14.79 ± 1.85 years; 60% male), as they have already completed the follow-up. The overall study is currently ongoing. According to Warnes’ classification, 29% of patients had simple heart defects, 46% showed moderate complexity, and 25% showed severe complexity. The comparison between T0 and T1 showed statistically significant improvement in T1 regarding pain/discomfort, anxiety/depression and perception of health status (EQ-5D), general satisfaction and quality of life (LAS QoL). Conclusions. The preliminary results showed in this study are encouraging, and confirm the need to create a multidisciplinary standardized intervention of education and support to deliver care for adolescent patients with CHD.
Razionale. La standardizzazione di un set di interventi educativi e di supporto di natura multidisciplinare, descritta come “Transition Clinic” (TC), è un concetto chiave dell’assistenza al paziente affetto da cardiopatia congenita durante la transizione dall’età pediatrica all’età adulta. Ad oggi, mancano evidenze empiriche a sostegno dell’efficacia di tali modelli. Per questo motivo, l’obiettivo di questo studio è valutare gli effetti del modello TC specifico per i cardiopatici congeniti adolescenti (TC CHD-specifico) introdotto nel nostro Centro, ponendo le basi per una solida validazione di questo modello gestionale. Materiali e metodi. Lo studio presenta un disegno quasi-sperimentale, tipo pre-post. La qualità di vita, la soddisfazione e la percezione di salute sono state indagate tramite una batteria di questionari, a T0 e ad un follow-up dopo 1 anno dall’arruolamento (T1). Durante questo periodo di follow-up, i pazienti arruolati (età 14-21 anni) sono stati coinvolti nel modello TC CHD-specifico. Risultati. Vengono presentati i risultati dei primi 100 pazienti (età media 14.79 ± 1.85 anni; 60% maschi) che hanno terminato il follow-up, poiché lo studio generale è ancora in essere. Il 29% dei pazienti presenta una cardiopatia congenita semplice, il 46% di moderata complessità e il 25% di severa complessità (classificazione di Warnes). Dall’analisi del confronto fra T0 e T1 emerge un miglioramento statisticamente significativo in T1 relativamente a dolore/fastidio, ansia/depressione e percezione dello stato di salute (questionario EQ-5D), e di soddisfazione generale e qualità di vita (questionario LAS QoL). Conclusione. I risultati emersi relativamente all’efficacia del modello TC CHD-specifico sono incoraggianti e confermano la necessità di creare un percorso standardizzato di transizione per i pazienti adolescenti con cardiopatia congenita.
