Splanchnic or peripheral hypo-perfusion or malperfusion may complicate, in most patients, type A acute aortic dissection (AAD) and it can create serious perioperative complications that significantly affect early and long-term prognosis. Particularly, malperfusion of lower limbs, both for static or dynamic flow alteration in AAD is reported in nearly 15% of patients.1 In the present report, we describe the case of a young male presenting with AAD and right leg ischemia due to flap occlusion of the right internal iliac artery treated with ascending aorta replacement and simultaneous femoro-femoral crossover.
