Over a 10-year period, seven patients with superior mesenteric artery (Wilkie's) syndrome have been observed and operated in a single institution. The pathogenesis of the disease is reviewed. Discussion is focused on diagnosis and results of surgical treatment. Duodenal section and duodeno-jejunal anastomosis seem to be a better procedure than simple by-pass.
