Objectives. A rare case of peripheral osteoma of the maxillary sinus associated with contralateral impacted canine is here presented. MATERIALS AND METHODS. Surgical removal of the lesion, followed by histopathological examination, was performed with the piezo-electric device. Cone Beam Computed Tomography (CBCT) was essential to plan a most conservative surgical treatment, thus avoiding any iatrogenic damage to the adjacent anatomical structures. RESULTS AND CONCLUSIONS. Immediate post-operative recovery occurred without complications and the panoramic X-ray taken one year later showed a good bone regeneration in the surgical site and lack of local complications. Osteomas are benign tumors characterized by slow development and absence of aggressive local pattern. Mostly asymptomatic, osteomas are generally diagnosed by routine or otherwise prescribed radiological examinations in young patients, within the fourth decade of life. The clinical importance of osteomas lies in their differentiation from a malignant lesion, such as the osteosarcoma, and exclusion of Gardner's syndrome as an underlying disease.
OBIETTIVI. Si riporta il caso di un raro osteoma periferico del seno mascellare sinistro associato alla presenza di inclusione del canino superiore destro. MATERIALI E METODI. La lesione è stata asportata chirurgicamente mediante l’ausilio del bisturi piezoelettrico e successivamente sottoposta a esame istopatologico. Importante nella programmazione terapeutica è risultata essere la tomografia computerizzata a fascio conico (Cone Beam Computed Tomography, CBCT) che ha permesso la pianificazione di un approccio chirurgico il più possibile conservativo e rispettoso delle strutture anatomiche vicine. RISULTATI E CONCLUSIONI. Il decorso postoperatorio si è svolto senza complicanze e l’ortopantomografia a un anno di distanza evidenzia la presenza di una buona rigenerazione ossea nel sito dell’intervento e l’assenza di complicanze locali. L’osteoma è un tumore benigno caratterizzato da una crescita lenta e dall’assenza di aggressività locale. Per lo più asintomatico, viene generalmente diagnosticato in modo incidentale dopo esami radiologici eseguiti di routine o per altri motivi in soggetti giovani, entro i 30- 40 anni. La rilevanza clinica degli osteomi risiede principalmente nella loro differenziazione da lesioni tumorali maligne come gli osteosarcomi e nell’esclusione della sindrome di Gardner quale patologia di base associata.
