Aim. Circular stapled hemorrhoidectomy according to Longo appears to be safe and widely performed by colon and rectal surgeons. Long-term follow-up is needed to determine whether initial results are durable. Methods. Two-hundred and seventeen patients with prolapsed hemorrhoids underwent stapled hemorrhoidectomy with a 33-mm circular device between April 1997 and November 2003. After a mean of 32 months (range 22-53) the first 86 patients were administered a standardised questionnaire by phone. Results. Sixty patients answered the interview. In 13 patients a secondary proctological surgery has been performed. Other 3 patients reported persisting episodes of prolapsing piles. Sixty-two percent was completely satisfied by the treatment and 82% answered that "will suggest to relatives of friends this technique". Conclusion. Stapled hemorrhoidectomy proved to be safe, effective and well tolerated by patients. Further experiences are needed to investigate recurrence, persisting skin tags, and the effectiveness in severe IV degree hemorrhoids with anal fibrosis.
