Classical procedure for groin hernia repair, namely anterior groin repair, does not consider the actual trend of the groin hernia surgery that is the preparation of hernioplasty without tension on suture line. Results of trials reported in the literature confirmed that the incidence of recurrent hernias is certainly higher in patients undergoing a traditional hernioplasty than is Bassini, Shouldice or Mc Vay procedure, in consideration of the long-term results obtained by tension-free surgical techniques such as Lichtenstein or Trabucco repair. Another innovation that has changed hernioplasty is the introduction of Marlex prostheses like polypropylene which doesn't increase morbidity after hernioplasty but gives the best adaptation as concerns tolerance and handling. The quality of material of new prostheses applied by new surgical techniques enables us to repair all types of hernia and to preserve the anatomical elements, sutureless repair with total absence of tension on suture line with a decrease or total absence, in some cases of typical postoperative complications of classic procedures.
