In 1991, on the basis of the Code of Marketing of Breast-milk Substitutes and the Ten Steps for successful breastfeeding, the international Baby-Friendly Hospital Initiative - BFHI started in order to encourage good practices for the promotion of breastfeeding and to help health-care services to improve care practices. The purpose of this study is to verify the trend of prevalence of obstetric practices related to three steps of the decalogue WHO / UNICEF for the recognition as "Baby Friendly Hospital" (4° -skin-to-skin, 6° -prevalence of exclusive breastfeeding , 7° -rooming-in) in the Obstetric and Gynecologic Clinic Mangiagalli Cà Granda Foundation – Policlinico Ospedale Maggiore in Milan in the years 2013-2014-2015-2016. The sample considered in this research is composed of 2141 mothers and their 2239 newborns. The 4° step - skin to skin contact, as defined by WHO / UNICEF, states that this will happen within 5 minutes after delivery and for at least 60 minutes, without any interference between the skin of the baby and the mother's skin. Over the years, the percentage of women in this category has been gradually decreasing from 18% in 2013 to 10% in 2016, far from 80% expected for accreditation. About the infant feeding, 46% of women in 2014 breastfed exclusively, choice shared by 65% of women in 2015 and 58% in 2016. Even these values must improve to reach 75% of infants exclusively breastfed during hospitalization, as expected by WHO / UNICEF. The application of the 7° step of the WHO / UNICEF decalogue was considered positive only for those mothers who have stayed with the baby for more than 23 hours out of 24: it is 34% in 2013, 35% in 2014, 46% in 2015 and 34% in 2016. Although they are more encouraging, the data about rooming-in are still below the 80% required by WHO / UNICEF. In order to promote breastfeeding and for the BFHI accreditation path, practices that reduce the separation of the dyad mother-baby after birth and during hospitalization could be implemented. Moreover, the assignment of personnel in each department could be rescheduled in order to ensure the presence of obstetric professionals dedicated to the monitoring of the newborn bonding.
