We investigated the prognostic value of any structural brain abnormality and of the location of the epileptogenic zone. A total of 228 pts with symptomatic localization related epilepsies, from 2 different centers have been included. All the pts included were studied with neuroimaging and EEG monitoring. Seizure control was achieved in 25% of pts. Brain abnormalities, especially Mesial Temporal Sclerosis, are a major prognostic factor. The location of the epileptogenic zone does not seem to be a determining factor. Seizure free periods during the evolution of epilepsy might be the cause of an unfavorable drug reduction with frequent relapses. In drug resistant cases the surgical options should be taken into account early.
