Aim: The purpose of this paper is to show the usefulness of Cone Beam Computed Tomography in the visualization of lesions and bone defects in the cranio-cervical-region. Materials and methods: 100 patients with Rheumatoid Arthritis underwent Cone Beam CT to evaluate the morphological abnormality of the condylar surfaces resulting from the disease. The sections obtained were studied in the three planes of space through the use of dedicated software 3Diagnosys. Results: The CBCT allows to extend our analysis to the relationship between the first cervical vertebrae and the base of the skull, otherwise no detail visible with conventional two-dimensional radiographic (panoramic radiograph, lateral cephalometric lateral and posterior-anterior). By observing these radiological investigations have been two cases of fusion of the occipital bone, atlas and odontoid process. Conclusions: This survey has been used by us to assess possible changes in bone in patients with rheumatoid arthritis. This chronic inflammatory disease is manifested in the joint. Therefore, the joint may present serious cranio-cervical abnormalities including a fusion of the occipital bone and vertebrae C1 and C2. This shows the importance of this diagnostic study, which allowed to place our attention on the anatomical structures are not strictly the responsibility of dentistry.

Scopi del lavoro: Lo scopo di questo lavoro è di mostrare l’utilità della Tomografia Computerizzata Cone Beam (CBCT) nella visualizzazione di lesioni e difetti ossei nel distretto cranio-cervico- facciale. Materiali e metodi: 100 pazienti affetti da Artrite Reumatoide sono stati sottoposti a TC Cone Beam al fine di valutare l’alterazione morfologica delle superfici condilari conseguenti alla malattia. Le sezioni ottenute sono state studiate nei tre piani dello spazio tramite l’utilizzo di software dedicati. Risultati: La CBCT permette di estendere la nostra analisi ai rapporti tra le prime vertebre cervicali e la base del cranio, altrimenti non dettagliatamente visibili con le tecniche radiografiche tradizionali bidimensionali (ortopantomografia, teleradiografia latero-laterale e postero-anteriore). Dall’osservazione di tali indagini radiologiche sono emersi due casi di fusione tra osso occipitale, atlante e dente dell’epistrofeo. Conclusioni: La CBCT è stata da noi utilizzata per valutare possibili alterazioni ossee in pazienti affetti da artrite reumatoide. Questa patologia infiammatoria cronica si manifesta a livello articolare. Pertanto, l’articolazione cranio-cervicale può presentare gravi alterazioni tra cui una fusione tra l’osso occipitale e le vertebre C1 e C2. Da ciò si evince l’importanza diagnostica di tale indagine, che ha consentito di porre la nostra attenzione su strutture anatomiche non prettamente di competenza ortodontica e gnatologica.
