Aim: Purpose of this study is to show the importance of the use of Cone Beam Computed Tomography (CBCT) for evaluation of joint damage in patients with Juvenile Idiopathic Arthritis (JIA), using volumetric measurements of the mandibular condyle. The JIA is a childhood disease that can cause damage in the temporo-mandibular joint and consequent alterations in facial growth. Materials and methods: 83 patients with articular involvement with JIA were submitted to CBCT. The mandible was isolated from other structures and has been segmented into several components, both right and left. The volumes obtained were evaluated with dedicated software. Results: The results show a very significant difference between the condyles affected and healthy tissue in patients with unilateral JIA (p <0.001), the difference between the volumes of the affected side branches and those branches of the healthy side was significant (p = 0,088). There was no significance in the comparisons performed between the volumes in healthy patients in the control group. Conclusions: The Cone Beam CT is a tool that helps in the diagnosis of joint damage, even in the earliest stages of the disease. The challenge for clinicians is the identification of patients with joint damage and their timely treatment, so as to prevent facial deformities typical of this disease

Scopi del lavoro: E’ mostrare l’importanza dell’utilizzo della Cone Beam Computed Tomography (CBCT) per la valutazione del danno articolare nei pazienti con Artrite Idiopatica Giovanile (AIG), tramite misurazioni volumetriche del condilo e della mandibola. Materiali e metodi: L’ AIG è una malattia dell’infanzia che può provocare danni a livello dell’articolazione temporo-mandibolare e conseguenti alterazioni nella crescita facciale. 83 pazienti affetti da AIG con coinvolgimento articolare sono stati sottoposti a CBCT. La mandibola è stata isolata dalle altre strutture ed è stata segmentata in diverse componenti sia a destra che a sinistra. I volumi sono stati valutati con software dedicati. Risultati: I risultati mostrano una differenza molto significativa tra i condili colpiti e quelli sani nei pazienti con AIG monolaterale (p < 0.001), la differenza tra i volumi dei rami del lato affetto e quelli dei rami del lato sano è significativa ( p = 0.088), non c’è stata significatività nei confronti eseguiti tra i volumi nei pazienti sani del gruppo controllo. Conclusioni: La CBCT è uno strumento che aiuta nella diagnosi di danno articolare, anche nelle fasi più precoci della malattia. La sfida per il clinico è l’individuazione dei pazienti con danno articolare ed il loro trattamento tempestivo, in modo da prevenire le deformità facciali tipiche di questa malattia
