Pulmonary bullae sometimes complicate Pneumocystis carinii pneumonia (PCP) and may rupture, causing pneumothorax. Because of the risks of surgery and the potential for developing concurrent infections, many established therapeutic approaches for removing these lesions are controversial in AIDS patients. We describe the successful use of sectorial pleurectomy under videothoracoscopy to remove a large, apical pneumatocele in an HIV-infected woman with PCP.
