A 71-year-old female presented with increasing dyspnea and atrial fibrillation and was found to have severe tricuspid insufficiency, pulmonary hypertension, an intact atrial septum, and a partial anomalous pulmonary vein connection (PAPVC) between the left superior pulmonary vein (LSPV) and the left anomalous vein (LAV) (Fig. 1A, B). At the time of surgery, the patient underwent tricuspid valve ring annuloplasty, bipolar ablation, and an anastomosis of the LSPV to the left atrium (Fig. 1C, D). The patient has remained asymptomatic and in normal sinus rhythm six months after surgery. Figure 1.
