In spite of its thickness and its strength, Achilles tendon rupture is relatively common among sportsmen over 40. As a matter of fact these lesions especially occur in 30-50-year-old males. Surgery, as soon as possible, is the golden treatment in order to allow a complete recovery of tendon functionality. Now different surgical techniques are available for reparation of chronic calcaneal tendon injuries: augmentation with the same tendon, reconstruction with autograft, with synthetic components or with allograft. We treated a young sportsman with chronic lesion of Achilles tendon inducing deficit in foot flex-extension, utilising Achilles tendon allograft. At 6-year follow-up the patient shows improved leg functionality and a return to sport activity. The excellent result of this treatment indicates allograft use as a relevant surgical alternative.
