Purpose: To evaluate the retinal function, the visual cortical responses and the neural conduction along the post-retinal visual pathways after treatment with Coenzyme Q10 in patients with open angle glaucoma (OAG). Methods:The results of 38 patients with OAG (mean age 51.8±3.7 ys, IOP < 18 mmHg with topical treatment, Mean Deviation of Humphrey 24-2 perimetry > -10 dB) were retrospectively evaluated. Simultaneous recordings of Pattern Electroretinogram (PERG) and Visual Evoked Potentials (VEPs) 1 were recorded at baseline and 6 and 12 months after treatment with Coenzyme Q10 and Vitamin E TPGS (Coqun ®, Visufarma, Italy, 2 drops 1 time/day, 22 eyes, GC Group), after treatment with Vitamin E TPGS (2 drops 1 time/day, 21 eyes GE Group) and after treatment with physiological solution (2 drops 1 time/day, 19 eyes, GP Group). Simultaneous recordings of PERG and VEP allow us to derive an electrophysiological index of post-retinal neural conduction that is called "Retinocortical Time (RCT)" 1. Results: At baseline conditions, similar values of PERG and VEP parameters (ANOVA, P>0.05) in GC, GE and GP eyes were observed. After 6 and 12 months of treatment, GE and GP eyes showed non significant (P>0.05) differences in PERG and VEP responses with respect to baseline conditions. In GC eyes it was observed a shortening in PERG P50 and VEP P100 implicit times and an increase in PERG P50-N95 and VEP N75-P100 amplitudes and the differences in implicit times and in amplitudes with respect to the baseline condition were significantly different (P<0.01) with respect to GE and GP ones. RCT was unmodified (P>0.05) in GC, GE and GP eyes. Conclusions: The results of this retrospective study suggest that Coenzyme Q10 treatment induces an improvement in retinal function as suggested by the enhancement in PERG responses. In presence of unchanged post-retinal neural conduction (unmodified RCT), the detected improvement in visual cortical responses (VEP enhancement) can be exclusively ascribed to the effect of Coenzyme Q10 in the retinal structures.
