The experience on 28 patients (22 with oligoasthenoteratozoospermia and 6 with teratozoospermia) who underwent microsurgical anastomoses from 1986 to 1988 is reported. Spermiogenic function and seminal plasma markers i.e. zinc, fructose, carnitine and transferrin were evaluated before and 8 months after microsurgical drainage. A significant improvement in sperm count (p<0.01), second hour motility (p<0.05) and in morphology (p<0.05) was observed in 22 patients with oligoasthenoteratozoospermia. No significant improvement of spermiogenic function was noted in 6 patients with teratozoospermia. Zinc and fructose showed normal values in all patients while carnitine value was below normal range in all cases and transferrin value was below normal values only in patients with oligoasthenoteratozoospermia. Postoperatively transferrin value improved at normal range (p<0.01). In conclusion the removal of the stagnant blood in the dilated veins of the varicocele by means of microsurgical anastomoses obtains a significant improvement of seminal parameters. Furthermore the normalization of seminal transferrin shows that microsurgery improves also tubular function.
