The discovery that apolipoprotein E (Apo E) ε4 allele is strongly linked to sporadic Alzheimer Disease (AD), raises the possibility that a dysfunction of the lipid transport system could be central in AD pathogenesis. The role of Apo E in the central nervous system is particularly important in relation to the cholinergic system, which relies on the integrity of phospholipid homeostasis in neurons. Recent evidence indicate that ε4 allele directly affect cholinergic activity in brain of AD subjects. These findings suggest that the isoform dosage of Apo E could be a prognostic indicator for the drug efficacy profile in the treatment of AD patients.
