Management of the Upper Third Molar Displaced in the Infratemporal Fossa : a Case Report

Purpose: The aim of this work is to present a case report of an accidentally displaced upper third molar in the infratemporal fossa. The surgery and the clinical evaluation of this rare situation are discussed and compared with the recent international literature. Methods: A young patient affected by upper third molar located in the infratemporal fossa was evaluated in this case report. Before surgery, radiographic examinations were done with the use of orthopantomograph, and computed axial tomography. Results: Antimicrobial prophylaxis was obtained with 1000 mg amoxicillin twice daily for 5 days starting 1 day before surgery. Topical anesthetic was used; mucoperiostal flap was elevated by electrosurgical device (Bladion ®) and after cortical bone was exposed. Then soft tissues dissection was performed by blunt scissor (Metzenbaum); tooth was founded and then extracted. Clinical Significance: The accidental displacement of the upper third molar in the infratemporal fossa caused by attempted extraction is a potentially condition in oral surgery. This case report wants the clinicians to evaluate the surgical technique and the management of this rare condition. Retrieval should be performed as soon as possible when this condition appears to avoid clinical complications and tissue reactions. OPT and TC images are important to point out the exact position of the impacted tooth and to accomplish the surgery.
