This chapter presents an integrative case formulation approach in psychotherapy, rooted in attachment theory and psychodynamic principles. The model emphasizes understanding patients’ interpersonal dynamics and intrapersonal factors such as attachment needs, affects, and defense mechanisms. Using the Triangle of Adaptation and Triangle of Object Relations, the chapter illustrates how early attachment experiences shape current relational patterns and emotional regulation. The approach aims to create a shared understanding between therapist and patient, guiding treatment plans and therapeutic goals. An exemplary clinical case is provided to demonstrate the practical application of this model, highlighting the importance of tailoring interventions to individual attachment styles and mental functioning levels.
