The essay first highlights the problems of the Italian system of care for non-self-suf- ficient elderly people: the lack of comprehensive state regulation; the failure to identify essen- tial levels in the field of non-self-sufficiency; the inadequate public assistance system. These issues have led to the adoption, over time, of numerous regional regulation and policies that are different with respect to the following aspects: the sector governance models and manage- ment responsibilities; the methods of providing service by Municipalities; the funding sources. Within this framework, the critical elements of regional and local regulations, policies, and care services for non-self-sufficient elderly people are examined. These critical issues have led to profound differences and disparities across the Regions with regard to the various planned interventions and the services actually provided. The essay then examines the innovative scope of Delegating Law No. 33/2023, aimed at reforming the elderly care system, and its subsequent partial implementation, in order to assess its impact on the local system of long-term care.
