The Italian health service is facing a structural crisis, worsened by years of budget cuts and lack of investment, culminating in the Covid-19 emergency. This scenario has triggered a wave of voluntary resignations among healthcare professionals, a phenomenon known as the “Great Resignation.” The main reasons, investigated in this paper, lie in inadequate pay also in comparative terms, unsustainable working conditions, lack of professional recognition, and a high psycho-physical workload. Young doctors, in particular, are the most likely to leave the National Health Service (SSN), often attracted by better opportunities in the private sector or abroad. The investigation also highlights the consequences of job insecurity and excessive bureaucratization, which contribute to the growing loss of trust in the public system. To address this phenomenon, structural interventions are proposed, including staff reinforcement, a better balance between care time and bureaucracy, and genuine recognition of the work performed, including its financial valuation. Without timely action, there is a real risk of the SSN being irreversibly weakened, leading to increasingly unequal access to care.
La sanità pubblica italiana sta attraversando una crisi strutturale aggravata da anni di tagli e mancati investimenti, culminata con l’emergenza Covid-19. Questo scenario ha innescato un’ondata di dimissioni volontarie tra i professionisti sanitari, un fenomeno definito come “Great Resignation”. Le motivazioni principali, indagate in questo lavoro, risiedono nelle retribuzioni inadeguate anche in termini comparati, nelle condizioni lavorative insostenibili, nella scarsa valorizzazione professionale e nell’elevato carico psico-fisico. I giovani medici, in particolare, risultano i più propensi a lasciare il Servizio Sanitario Nazionale (SSN), spesso attratti da opportunità più favorevoli nel settore privato o all’estero. L’indagine qui condotta sottolinea le conseguenze del precariato e dell’eccessiva burocratizzazione, che contribuiscono alla perdita di fiducia nel sistema pubblico. Per arginare il fenomeno, vengono proposti interventi strutturali, tra cui il rafforzamento del personale, un riequilibrio tra tempo di cura e burocrazia, e un riconoscimento reale del lavoro svolto, anche sotto il profilo economico. Senza azioni tempestive, il rischio concreto è l’indebolimento irreversibile del SSN e un accesso alle cure sempre più diseguale.
