The aim of this work is to show an unusual clinical case about the lower lip paresthesia occurred during an orthodontic treatment after the lower right molar distalization. During the treatment with the fixed appliances. The radiological exams showed an excessive growth and length of the roots of the lower second molars that, coupled with the distalization, had lead to an interference with the mandibular canal and a compression of the mandibular nerve. Lower paresthesia during molar distalization is a very unusual and rare. When a distalization of the lower second molar is requested, we suggest to make a detailed and closer observation of the panoramic radiograph in order to avoid the possible risks. In the case of symptoms’ rising, a computerized axial tomography (CAT) is useful to evaluate the situation. The treatment consists both of an immediate interruption of the forces applied to the lower second molars together with pharmacological therapy.
