Aim: The ultimate goal of the orthodontic-surgical treatment of dysgnathia is the treatment of all components of the stomatognathic system, dental, skeletal and neuromuscular. Materials and Methods: Orthodontic treatment before and after surgery should lead to the alignment of the teeth on the arches in a harmonious relationship with the future spatial position that the foundation will acquire maxillary bone surgically. The surgical approach of both jaws must be aimed at the three-dimensional correction of dysgnathia, with particular regard to the correction of skeletal vertical dimension. The displacements surgical in the three dimensions of space, programmed on the basis of cephalometric data and models, must be carried out in harmony with the neuro-muscular component for obtaining an excellent final result from the functional point of view. Results: To achieve these objectives, our school makes use of electromyography and kinesiography, which allow us to analyze neuromuscular function, completing a three-dimensional correct diagnosis, and to proceed in the appropriate therapy. Conclusions: Such means confirm the effectiveness of the corrective treatment done and the optimization of the end result, thus eliminating the conditions for the reappearance of relapse and the establishment of an iatrogenic disease, particularly at the level of the temporomandibular joint with organismic repercussions.
