Aim of the work: The present work aims to highlight how the close collaboration between orthodontist, periodontist and hygienist is fundamental in the management of the adult periodontal patient. Materials and methods: in this work we want to show a paradigmatic patient. He is an adult patient, today classified as stage III grade B, he came in office for the migration and extrusion of 2.1. On the periodontal examination there was a pokts with a depth greater than 5mm. He was inserted in the periodontal causal therapy. after the causal therapy, at the time of the re-evaluation the orthodontic treatment was proposed for the complete repositioning of the dental elements. Conventional orthodontic therapy is combined with the stripping technique proposed by Sheridan and periodontal support therapy every three month for the total duration of the treatment. By mean of the stripping technique space closure is obtained with anatomical modification of the dental connectors and closure of the probing angle by orthodontic displacement with healing by long junctional epithelium attachment. Results: the close collaboration within the team has allowed to obtain a correct repositioning of the dental elements, to allow a simplified support therapy thanks to the power of self-dersion given by the alignment and to avoid periodontal procedures. Conclusion : orthodontic therapy associated with an adequate relationship between orthodontist, hygienist, patient and periodontal may be associated with causal non-surgical therapy in the resolution of periodontal pokes not exceeding 5mm. Of fundamental importance is the retention which, in case like this, it is better that it is fixed but able to allow the maneuvers of hygiene both at home and professional. In our experience, retention is carried out through ECSMR(Extra Coronal Steel Mash Retainer) this is functional to the management of the patient by all the players in the team.
