A series of 21 major pulmonary resections performed between October 1991 and November 1992 is reported. The series included two left pneumonectomies, 18 lobectomies and one segmentectomy. This experience represents part of a large series of 157 thoracoscopic interventions performed in the same period. The criteria for major pulmonary operations were limited to three indications: 1. benign pulmonary diseases requiring lobectomy, e.g. localized purulent bronchiectasis (four patients) and an arteriovenous fistula causing severe haemoptysis (one patient). 2. Endobronchial pulmonary metastasis which were not removable by classic wedge resection and therefore requiring lobectomy (two patients) when the primary extrapulmonary neoplastic site was under control. 3. Primary lung cancer at stage I requiring lobectomy (12 patients). Lymphadenectomy was performed in each case. In two cases histology revealed stage II (pT1,N1) disease. Left pneumonectomy was performed videothoracoscopically in two casesof small tumor involving the distal end of the left main bronchus and not susceptible of sleeve lobectomy.There were neither major operative complications nor any deaths. Five further patient intended to undergo videothoracoscopic resection were converted to open procedure because of intraoperative technical problems.
