Background: moral distress (MD) can influence nurses’ wellbeing on workplace, and is often associated with burnout and intention to leave. It has been studied in many settings, but no studies are available in correctional medicine, and no specific tools are available. This study aims to create and validate a tool for assessing moral distress among nurses working in a sample of Italian correctional facilities. Materials and methods: The Moral Distress Scale for Correctional Nurses (MDS-CN) was created. Face and content validity were assessed by a pool of expert nurses through the Content Validity Index. The scale was administered to 60 nurses working in the facilities of Bollate, Opera, and San Vittore (Milan). Internal consistency and test-retest stability were assessed by means of Cronbach’s alpha and Spearman’s rho coefficients. Results: The Content Validity Index-S was 0.99; test-retest reliability was excellent (rho=0.987, p<0.001). So was internal consistency (alpha=0.91). The median score of moral distress in the sample was 40.5/100. Working experience in correctional facilities was strongly associated with moral distress (OR=4.571, 95%IC=[1.452;14.389], p=0.009). Conclusions: The MDS-CN is a valid and reliable tool for assessing moral distress among nurses working in correctional facilities. The study should be repeated on larger samples, to allow factor analysis. This pilot study is the first Italian study on this topic and highlights the relevance of moral distress in this setting.
