Acute pancreatitis has a clinical course which ranges from mild self-limiting in about 80% of cases to severe. Local and systemic complications as well as mortality and need for surgery occur only in severe disease which is usually characterized by the presence of pancreatic necrosis. Biliary disease and alcohol abuse represent the most common causes of pancreatitis. Therapy of pancreatitis depends on the severity and etiology of disease. Protease inhibitors (gabesate mesilate), total parenteral nutrition and antibiotic prophylaxis are usually used in the treatment of severe pancreatitis, while surgery is limited to pancreatic infections and local complications.
