AIM: To analyze the association of p53 overexpression in index colorectal adenomas with metachronous adenomas development. METHODS: A total of 46 patients who underwent endoscopic polypectomy were followed by complete colonoscopy every six months for four years to remove any metachronous adenomas. All adenomas removed were immunostained with a monoclonal antibody against the p53 protein on formalin-fixed, paraffin-embedded materials. RESULTS: Among the 46 patients studied, 16 developed metachronous adenomas (group I), while 30 were free of recurrence (group II). The overexpression of p53 was significantly higher in group I than in group II. CONCLUSIONS: Immunodetection of p53 overexpression might help to identify the group of patients where the risk of recurrence is the highest.
