Aim of this letter is the proposal to use enteral Tolvaptan in case of free water absorption in endoscopic surgery. We report a case of profound hyponatremia after transcervical myomectomy, due to irrigant solutes’ absorption (2.7% sorbitol/0.54% mannitol solution, containing free water 178 mOsm/L) causing volume overload, dilutional hyponatremia and possibly pulmonary/brain edema.
