Aims: The aim is to show the role of orthodontists to detect psoriatic arthritis before systemic and craniofacial complications. Materials and Methods: Psoriatic arthritis (PA) is an inflammatory arthritis associated with psoriasis. The most common subjective symptom affecting the masticatory system in patients with PA is pain from the TMJ area during function. Typical clinical signs frequently found in patients with PA are tenderness to palpation of the TMJs and masticatory muscles and TMJ crepitation. The most common radiographic finding is erosion of the cortical outline, which is a typical sign of inflammatory TMJ involvement, easily identified by Cone Beam TC. Results: Psoriatic Arthritis is a disease not easy to detect. In this work a psoriatic arthritis case has been reported, in which the disease has been diagnosed after several years from temporomandibular onset, because no other sign was present apart from psoriasis. The lost early diagnosis has caused a severe TMJ damage. In other cases the disease has been early detected using Cone Beam TC. Conclusions: The TMJ could be the first involved articulation by PA. For a correct and early diagnosis of PA, an interdisciplinary collaboration between orthodontist and Rheumatologist it is very important.
