A 26 year old woman with squamous cell cervical carcinoma implanting in the episiotomy site is reported. Two months after delivery she presented with a painful, hard, ulcerated tumour in the episiotomy site. Pelvic examination showed a large primary lesion involving the cervix. A pathological diagnosis of cervical carcinoma with an implant in the episiotomy site was established and the patient received external beam irradiation and brachytherapy. No response to radiotherapy was observed, and six cycles of cisplatinum, methotrexate plus leucovorin and bleomycin were administered resulting in only analgetic effect and stabilization of the carcinomatous lesions. The patient died of rapid progressive disease 12 months after the diagnosis had been established. Other 11 previously reported cases of cervical cancer implanting in the episiotomy site are reviewed. The inadequacy of cervical cytological screening in pregnancy and the need for careful inspection of the vulva and perineum in patients with invasive cervical cancer who have recently delivered vaginally is strongly stressed.
