The authors evaluate the distribution of lymphatic dissemination in gastric cancer. From January to November 1990 24 patients underwent curative gastrectomy for gastric cancer. A radical R2 lymphadenectomy according to the Japanese Research Society for Gastric Cancer was always performed. Lymph node involvement was observed in 58% of all cases. About one third of the cases presented the second node groups involved. No significant relations were observed between the size of the primary tumor, its site and histological type and the rate of nodal involvement. Lymph node invasion significantly correlates with the degree of tumor penetration of the gastric wall. The frequency of involvement of the different node groups seems to depend on the location of the primary tumors.
