Lung recruitment represents a crucial aspect of the ventilatory management of patients affected by acute respiratory distress syndrome (ARDS). In the last years, we have clearly demonstrated that: 1) lung recruitment depends on the level of end-inspiratory airway pressure applied: 2) lung recruitment depends on the level of PEEP applied: 3) lung recruitment strictly depends on the maximal potential for lung recruitment. These concepts may found the bases for an optimal ventilatory strategy of ALI/ARDS patients.
