In the period 1986-1989 the Authors have performed 37 reduction mammoplasties with the central breast pedicle technique presented by Hester in 1985. The advantages of this technique are the rich blood supply to both the glandular pedicle and the skin flaps, which has allowed for remarkable superior movement of the nipple in the harder cases, and the sculpturing of a conical gland with a nipple completely free from dermal connections, which yields nicely shaped breasts and excellent nipple areola projection. Complication rate was low and in particular hypertrophic scarring was experienced in 6 cases only, possibly because of skin removal being performed after breast reduction with consequently lessened tension. No skin flap or nipple areola necrosis were recorded. This method, in the opinion of the Authors, is a valid alternative to the more popular breast reduction techniques utilized today (inferior and superior pedicle, vertical bipedicle).
