Objectives: The aim of this work is to introduce the protocol used at the Orthodontic Department of Milan University. Materials and methods: All diagnostic and laboratory steps are described. Results: The occlusal splint guides are made on a cast model after developing an orthodontic set-up. At any appointment the clinician can evaluate if the occlusal guides fit and what dental movements are necessary; this is possible without repeating records and taking new impressions. The evaluation of orthodontic treatment progression is also possible whereas undesirable effects, such as jiggling movements, can be avoided. The objectives of presurgical orthodontics are achieved when occlusal splint guides fit perfectly: at this moment the patient might be considered ready for surgery. Conclusions: The protocol described by the authors allows to obtain an accurate orthodontic planning before surgery and to optimize treatment steps with consequent advantages in clinical practice.
Lo scopo di questo lavoro consiste nel descrivere il protocollo in uso presso il reparto di Ortognatodonzia del Dipartimento di Scienze Chirurgiche, Ricostruttive e Diagnostiche dell’Università degli Studi di Milano. Materiali e metodi Sono presentate tutte le fasi del protocollo, con i relativi vantaggi. Risultati Tra le varie fasi del protocollo particolarmente significativo è l’utilizzo di mascherine termostampate, costruite sulla base di un modello dove è stato effettuato il set-up ortodontico. Il clinico potrà valutare a ogni appuntamento quanto calzeranno le mascherine nella bocca del soggetto. Potrà quindi stabilire l’entità degli spostamenti dentali da effettuare evitando di ripetere più volte il rilievo dei record (rilevamento impronte) per valutare il raggiungimento degli obiettivi della fase di ortodonzia prechirurgica ed evitando fenomeni indesiderati di jiggling. Il raggiungimento degli obiettivi della fase prechirurgica può essere ottenuto quando esse calzano perfettamente e a tal punto il paziente può considerarsi pronto per subire l’intervento stesso. Conclusioni Il protocollo descritto consente di ottenere una programmazione ortodontico-chirurgica estremamente precisa e di conseguenza ottimizzare le varie fasi del trattamento, con significativi vantaggi in ambito clinico.
