Congenital Permanent Dislocation of the Patella is a condition in which the patella is permanently displaced and fixed, without trauma, on the lateral aspect of the lateral femoral condyle due to patellofemoral malalignment or congenital trochlear dysplasia. Its etiology is not well understood. Early diagnosis and definitive correction are recommended to prevent subsequent disabilities as well as degenerative changes of knee joint This condition is rarely diagnosed at birth and radiographically early diagnosis is difficult because patella does not ossify before 3-5 years of age or later if dislocated. Conservative treatment does not solve the problem because dislocation is permanent; a surgical approach is compulsory. This paper describes a case of a young patient affected by permanent dislocation of the patella and right lower limb ipometry, treated by modified Roux-Goldthwait technique and two years later by tibial lengthening to correct dismetry
