In the manuscript entitled "Assistance to physiciancs working in remote places. Initial experience with telematic approaches: Pointe Noire and Linosa" we refer to two experiences which can supply sound elements for reflection and consideration in respect to the practical potential applications of telematic medicine. The first experience described regards the long-distance shared clinical management of patients in the area of a research project carried out between Milan and Pointe Noire (Congo). Assistance for the doctors operating in remote places without no chain of clinical back up was the reason that this project came about. The second refers to a trial carried out on the island of Linosa (0.55 sq miles) aimed at determining multifactorial cardiovascular risk in a confined population. Among the aims of this study was to verify the possibility of using telemedicine methodology to send biological data recorded on site to a specialised centre to be elaborated at a later date. Telemedicine and health, above all with a new emphasis in prevention, are becoming a stable connection, which requires rethinking, of the healthcare process determining drastic redesigning of the diagnosis, care and prevention.
