The Authors report ten cases of complicated large incisional hernias, that have been operated according to different techniques, using two prostheses. Particularly, they report their personal technique, used in cases of large incisional hernia and in other abdominal wall diseases, where there is an important loss of soft tissue. In these cases, two prostheses are used at the same time. A slow-resorption mesh is sutured to the peritoneum, in direct contact with the previous one, or in the retromuscular space. The retentive function is good, whereas the chest-wall functionality, examined by trans-diaphragmatic manometry, is not complete. The Authors consider the cause of this to be the incomplete reinsertion of broad muscles, due to interposition of the slow-resorption mesh to close the defect.
